SeniorBridge is hiring a Supervisor of Care Management in Jacksonville, FL.
SeniorBridge is Humana's wholly owned, Private Pay home care business acquired in 2012. At SeniorBridge, we're passionate about helping families and loved ones find solutions that make life better. We provide concierge care management, private duty nursing, and personal care assistance services to seniors.
The Care Management Supervisor provides clinical/administrative supervision and leadership to the care managers on their team. This clinical leader assists the Branch Manager in the ongoing clinical training/supervision needs of staff, helps achieve compliance with regulatory agency standards, and support the overall goals and objectives of the organization.

Responsibilities
The position is responsible for ensuring quality and appropriateness of service to all clients, and for maintaining a high level of client satisfaction. The position will be knowledgeable and compliant with all applicable laws and regulations to which we may be subject. By ensuring service and satisfaction standards, the position spearheads high levels of 'Word of Mouth' referrals and budget goals.
Authority and Responsibilities
Depending on the size of the office, the Supervisor of Care Management may directly perform some or all of the activities associated with the responsibilities below. The position includes, but is not limited to, the following:
Ensures Clinical competence/excellence
Responsible for maintaining Professional/Paraprofessional productivity and margins within budget goals
Responsible to maximize services for revenue growth by reviewing and approving all cases and potential care plans
Ensures superior customer service
Adheres to all policies, procedures and standards set forth by the company, state and federal regulations
Anticipates, identifies and reports in a timely manner on areas of potential risk; and plans their resolution or prevention. Consults with the Manager and Corporate Leadership staff as appropriate.
Must be available to work in the field
Must be able to participate in our on-call rotation
Must be able to occasionally travel to other branches
Required Qualifications
Valid RN License in the state of Florida with no restrictions/disciplinary actions
Minimum of 1-2 years of supervisory experience
3-5 years of nursing experience, preferably in care management
Existing community and professional contacts within the geographic market
Proven leadership, ability to influence, motivate, develop, and empower employees to achieve objectives with a team approach
Proficient with Microsoft application such as: Outlook, Word, Excel, and Power Point
We will require full COVID vaccination (**** for this job as we are a healthcare company committed to putting health and safety first for our members, patients, associates and the communities we serve. This job requires close personal interaction with patients. You will not be able to social distance.
If progressed to offer, you will be required to provide proof of full vaccination where allowed by law. Medical and religious exemptions will be considered on a case by case basis.
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB and pass a physical
This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits

Preferred Qualifications
BSN or Master's degree in Nursing
Home infusion experience
Proven ability to lead teams and build strong working relationships, internally and externally
Experience leading remote multi-location teams
