UI/ UX Designer to design form to app
I have a Google form I want covered into an App. Design the front end of the app and back end if needed.
Need to create an event activity tracker. Users will add events they attend on the front end and be able to upload a document to each activity if needed. Think type form etc. Proficiency with excel is useful so they can see a list of activities they have completed throughout the year. A typical form will have the following: Start date, End date, Activity Name... etc.
