Key Responsibilities


Delivers personal care to patients in a caring and respectful manner as directed by the RN/LPN and established plan of care:


Delivers direct patient care as delegated, checks with RN/LPN for changes in care, explains to the patient what s/he is doing, seeks assistance when unable to answer patient’s questions, frequently check with patient to determine that s/he is comfortable, and maintains privacy while doing procedures. 

Communicates and works cooperatively with RN/Chg/LPN by offering suggestions, reporting observations and patients’ response to procedures, and providing pertinent information for shift report.

Documents accurately and timely (at time of occurrence) in the medical record, vital signs, I & O, weights, procedures (enemas, etc.), personal care (baths, skin care, etc.), ADLs, specimens as directed, observations, diet intake, and ambulation.

Performs tasks for all age groups following specific instructions or RN/LPN.

Seeks assistance for new/unfamiliar skill/task that is delegated.

Maintains competency in clinical skills for service to which assigned:


Demonstrates competence in the performance of selected skills and knowledge on an annual basis.

Routinely performs selected tasks/skills and aspects of patient care (skills, ADL, treatments and use of equipment) in a timely and safe manner according to established standards, policies and procedures.

Seeks assistance for new or unfamiliar skills.

Assumes routine coordination of clerical activities under the direction of the RN/Charge Nurse (omit this section if this role does not apply):


Acts as a communication liaison between co-workers and other departments, reports test results when obtained, notifies staff on new/stat orders, reports information received from other departments/staff, reports unit problems to charge nurse.

Checks stock supplies on a routine basis to assure consistent adequate supplies.

Promptly locates charts and other materials when requested.

Completes clerical tasks accurately and timely, admission, discharge, transfer of patient.

Transcribes orders accurately and according to policies.

Assists director in other related duties as required and assigned.

Job Requirements/Qualifications


Nurse Aide Training.

Registration through ND Board of Nursing or State Board of Health.

Capable of meeting the physical and emotional demands of the position.
