OverviewDemonstrates the knowledge and skills necessary to manage care of clinic patients in a Patient-Centered Medical Home. Works in collaboration with a physician and other members of the interdisciplinary team to evaluate and treat illnesses and injuries, ordering, conducting and interpreting appropriate diagnostic and laboratory tests, prescribing of pharmacologic agents and treatments, and performing in office procedures per procedures, policies, and evidenced based practice.  Provides expert education, consultation, and patient advocacy. The Community Health Care Clinic is committed to the Patient-Centered Medical Home model of care and expects each staff member to embrace their role within this model. Qualifications REQUIRED QUALIFICATIONS: Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. Licensed to provide either an advanced practice nursing services in the State of Illinois as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Nurse Practice Act] OR [physician assistant services as issued by the Illinois Department of Financial and Professional Regulation in accordance with the Illinois Physician Assistant Practice Act of 1987 (Act) Licensed by the Federal Drug Enforcement Administration and State of Illinois for prescribing of narcotics.
