Simple web form and web page development in two stages on WIX:
1) Stage 1 - Proof of concept. See what is possible on WIX. We need to have built a simple 10 question form that feeds into a unique password protected dashboard that gathers information from all the submitted forms.
2) Once proof of concept is finalized - move to recreate approx. 10-12 instances (i.e. 12 unique login pages for form submission.
Use case and details:
Use Case: Our clients need to have their employees complete a short health questionnaire/form on a daily basis. The client administrator then needs to login to a unique password protected page that has a dashboard with all the related form submissions, sorted on a daily basis. That dashboard should be able to search and sort by employee, and show scores from form, and also who has submitted a form each day versus those that have not.
Form requirements: For will have approx. 10 questions related to health/COVID-19 questions. Employees will submit them on a daily basis as a requirement. Once submitted, this form will be recorded on a password protected webpage unique to the clients employees. Each form will require unique identification for each employee and company - which can be done ideally by user email and company domain. Each form will have a pass/fail score based on answers to questions.
Webpage/Dashboard Requirement: Each client will have a unique, password protected webpage where the form submissions can be reviewed and sorted by day, employee, and form results/score. The client admin needs to be able to see the following every day for each employee:
1) Who took the test/submitted the form
2) Who failed the test and or score was below required
3) Who did NOT take the test and or submit the form
Pages/forms MUST be mobile optimized as many will be done on phones.
