Do you want to be part of a larger purpose and an evolving, high-performance culture that empowers you to make an impact?nu00a0nNurse Care Manager I/IInLocation: Work from home.u00a0 CA, NV, TX, OH, TN, KY, MO, ME, NJ, NY, VA, IN, GA, FL - must live within 50 miles of an Anthemu00a0officenHours: u00a0Operation hours are M-F 8:00am to 8:00pm (depending on time zone), but candidates will work a 8 hour shift which will be determined.u00a0 May include rotating weekends.nu00a0nHow you will make an impactnResponsible for performing telephonic care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. nPrimary duties may include, but are not limited to:nntEnsures member access to services appropriate to their health needs.ntConducts assessments to identify individual needs and a specific care management plan to address objectives and goals as identified during assessment.ntImplements care plan by facilitatingu00a0authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements.ntCoordinates internal and external resources to meet identified needs.ntMonitors and evaluates effectiveness of the care management plan and modifies as necessary.u00a0ntInterfaces with Medical Directors and Physician Advisors on the development of care management treatment plans.ntAssists in problem solving with providers, claims or service issues.nnQualifications Nurse Care Manager InMinimum Requirements:nntRequires minimum of 3 years of acute care clinical experienceu2026 ER, ICU, Med/Surg; or any combination of education and experience, which would provide an equivalent background.ntCurrent, unrestricted RN license in applicable state(s) required.ntCurrent, unrestricted CA RN license required.ntAbility to obtain RN multi-state licensure is required if this individual is providing services in multiple states.ntExperience with electronic documentation ntnKnowledge and work experience with Microsoft Office (Excel and Outlook)u00a0nnnPreferred Capabilities, Skills and Experience:nntBilingual a plus.ntAbility to type 45 wpm with minimal errors.ntCertification as a Case manager is preferred.ntTelephonic Case Management experience a plus.ntAS or BS in nursing preferred. ntPrior managed care experience preferred.ntPrior experience with Diabetes educationntPrior experience using digital platforms and electronic monitoringnnu00a0nNurse Care Manager IInMinimum Requirements:nntRequires minimum of 3 years of acute care clinical experienceu2026.
