Job Requirements
Responsibilities of LPN's:
Provide personal care/incontinence care, administer medications and feeding through GI tube
Care for individual based on personalized “Plan of Care”
Informs the Director of Nursing and any other appropriate personnel of changes in the client’s conditions and needs
Counsels client and family through care process while updating family on a regular basis
Assures proper maintenance of clinical records in compliance with local, state, and federal laws
Requirements:
Minimum of 3 years of documented in-home care/facility experience is required
Current unrestricted LPN license in the state of Pennsylvania
Valid CPR Certification
Recent TB test: 2-step skin test, blood test or chest x-ray
Reliable car/driver’s license/auto insurance
Criminal background check and drug screen prior to starting work
Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
Must be able to lift up to 50 pounds.
