Description

SeniorBridge is offering PRN work with flexible hours, including days, evenings and weekends. Need is for 10-15 hours per week.

We are seeking Private Pay Home Care RNs dedicated to delivering the highest quality in-home care management.

SeniorBridge is Humana's wholly-owned home care business acquired in 2012.

Responsibilities

As RN Home Care Nurse you will direct care to clients and their families using a multidisciplinary approach. The RN Home Care Manager position includes the following:


Conducting admissions and assessments to identify problems and necessary services
Developing a professional care plan from the assessment findings and in collaboration with the clients Physician
Ensuring in-home care delivery is in accordance with each client's contractual program requirements and is high quality and complies with company-adopted, evidence- based clinical practice guidelines
Thorough and timely documentation of initial and ongoing assessments and outcomes
Communicate explanation of the care plan and all subsequent cost of all services to client and or Families, Attorney or Guardians
Act as a liaison to families as well as with collateral clinicians, professionals & agencies.
Observes infection control practices and uses standard precautions
Participates in quality management/performance improvement activities


Schedule: Must be available up to 15 hours per week and participate in the on-call rotation. On-Call is Monday-Thursday 5PM-8:30AM; Friday 5:00PM-Monday 8:30AM.

Required Qualifications


Registered Nurse with 1 year of experience of in home case/care management
Experience working with the adult population
Knowledge of community health and social service agencies and additional community resources
Ability to travel to member's residence if needed within 30 to 40 miles
Exceptional communication and interpersonal skills with the ability to quickly build rapport
Ability to work with minimal supervision within the role and scope
Ability to use a variety of electronic information applications/software programs including electronic medical records
Excellent keyboard and web navigation skills
This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Valid driver's license, car insurance, and access to an automobile
Must be passionate about contributing to an organization focused on continuously improving consumer experiences


Preferred Qualifications


BSN
3-5 years of in home assessment and care coordination experience
Experience with health promotion, coaching and wellness
