Valid HHA or PCA certificate
Valid ID (NY State ID / Passport / Resident Card)
Social Security card
Two (2) typed references
Medical Requirements
Physical (cannot be older than 6 months)
Drug screen test (cannot be older than 6 months)
Rubella, Rubeola, & Varicella blood work (from the lab)
PPD or Quantiferon (cannot be older than 3 months
** (If the PPD is positive then a chest x-ray is required)
