Responsibilities

Conviva is seeking an Administrative Support Assistant for Care Management. Candidate must be detail oriented, flexible, dependable, with strong written and verbal communication skills.

The Administrative Support Assistant provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Administrative Support Assistant performs varied activities and moderately complex administrative/operational/customer support assignments. Typically works on semi-routine assignments. Must be passionate about contributing to an organization focused on continuously improving consumer experiences

This position will cover the Palm Beach market, travel is required.

Job Functions

Assist Care Managers with coordinating the care plans for patients.

Assists patients with needed resources, government benefits and referrals.

Follow-up with patient calls, responding to letters, and helping them live their best life.

Required Qualifications

Administration support experience with DCF or a healthcare industry

Must be Bilingual English (read, write & speak) and Spanish (speak)

Must be able to travel within Palm Beach County city limits

Preferred Qualifications

Associate's Degree

Proficient with Microsoft Office applications including Microsoft Word, Excel and Outlook.

Experience with EMR system

Experience with Medicare and/or Medicare Advantage

Working Hours: Monday - Friday, 8AM - 5PM
